The Community Health Needs Assessment (CHNA) aimed to identify and prioritize the health needs of our community, the Fort Defiance Indian Health Board, Inc. (FDIHB, Inc.) Service Delivery Area (SDA). The process leveraged a comprehensive methodology that integrated both primary and secondary data sources. Primary data was gathered from the hospital’s registration and patient management systems (RPMS), which provided health trends and patient demographics. Additional primary data included results from surveys, hospital operations reports, and insights from community focus groups utilizing the nominal group technique to ensure a broad range of perspectives.
