Fort Defiance Indian Hospital Board, Inc. (FDIHB) established the Mobile Health Program, which provides care in some of our outlying communities on a rotating basis. It is a bridge to an overburdened Primary Care Clinic and provides increased access to care. Local community centers called “Chapter Houses” provide parking facilities, and we work closely with Public Health Nursing, Chapter Houses and hospital leadership when local health issues are emergent.
On the 2nd Wednesday of each month, this clinic travels to various chapter houses and provides a range of screening and education services to older community members, including eye health checks, hearing screening, immunizations against flu, shingles, and pneumonia, balance assessments and fall prevention, traditional healing, oral health education and screening, primary care visits, and medication reconciliation.
Staffed by a Family Nurse Practitioner or Physician Associate and a Certified Medical Assistant, this clinic is open Mondays through Thursdays at rotating community locations. We serve men and women, and children ages 3 and up. Services include: primary care, diabetes screening and management, flu shots and other immunizations, physicals, treatment of minor illness, HIV/STI testing, and more.
Parking at the Window Rock High School typically on Mondays, this clinic is operated by staff from Mobile Health Program to provide students with comprehensive services including: physical exams, health education, immunizations, reproductive health, treatment of minor illness, and mental health screening and referrals. We aim to be a resource for students to discuss any questions or concerns in a safe and confidential space, and to educate about and inspire careers in healthcare.
Providing a full range of services at select chapter locations to complete school and sports physical requirements; staffed by Pediatrics, Dental, Optometry, and Rehab.
